As a Patient Centered Medical Home practice we strive to communicate effectively with our patients and provide them with the ability to take an active role in their care.

What is a Patient Centered Medical Home?

We follow the AMA guidelines for age related care/health maintenance/behavioral health and refer to our local panel of specialists and hospitals when needed. Whatever your healthcare needs we are here to provide you with guidance and support. 


Patients are cared for by a team of highly trained professionals consisting of Board Certified Physicians and mid level Practitioners, Registered Nurses, X-Ray and Lab Technicians, and administrative support staff all trained to provide quality and compassionate care. Patients can expect evidence-based care from their clinician and team, as well as support for self-management of their health and health care.

 

To effectively serve as a medical home, the practice must have patient information such as medications; visits to specialists; medical history; health status; recent test results; self-care information; and data from recent hospitalizations, specialty care or ER visits.

1. Clinical Summaries

Clinical Summaries will be given to every patient upon discharge with medications including changes, treatment plans, lab and diagnostic orders, and if indicated patient care plans withself monitoring and treatment goals. 

2. Patient Record Access

The Patient Portal provides secure online access to your chart 24 hours a day. Electronic records include visit summaries, medications, medical history, diagnostic and laboratory tests, current treatment including patient care plan.

3. Same day appointments

Routine, emergent and sick appointments, are available every day in addition to after hours walk-in services. All walk-in patients are welcomed.

4. Reminders

You will be reminded of your appointment by an automated process. The automated system will also call to remind you about overdue labs, follow-up office visits, and other health maintenance items. Please arrive 15 min prior to your appointment. If you arrive late, you may be rescheduled.

5. Clinical Advice during business hours

During business hour, you can call our staff with any questions. They will document your call in your chart and forward it to the appropriate provider or staff. Messages will be answered by the medical team within a timely manner.

6. Clinical Advice after business hours

If an Emergency, please call 911 or go to nearest emergency room. If seeking medical advice an answering service is provided giving information and forwarding calls to the on-call physician.

7. Tests and Imaging

Orders are generated electronically and can be transmitted to most places electronically as well. A paper copy will also be given to you. Please schedule a follow up appointment within 1 week to discuss your test results.  If you require assistance with scheduling your test, our staff can assist you with this process.

8. Prescriptions

Your prescriptions will be sent electronically to your pharmacy or given to you during your office visit. If you do not understand why or how to take your medication please ask and our staff will explain any questions you may have. It is best to schedule your appointment 2 business days before your renewal is needed. If you have a reaction to or have a problem with your medication call our office immediately or proceed with emergency care.

9. Referrals

Our team will assist the patient in setting appointments, transferring records including labs and imaging results to our preferred panel of specialists and hospitals. Prior authorization required by insurance companies will be handled by our referral coordinator. 

10. Records Release

All practices require that Authorization for Release of Health Information Pursuant to HIPAA guidelines be completed for your chart. Records will be transferred for transitional care for continuity of care when the referral is made.
