Patient Portal

You have access to your medical records online, pay bill, request refills, and can send secure messages to your provider through the Patient Portal. 

Instructions to get online access to your Medical records and Questionaire:

  1. You can log in using your mobile phone associated with your name.
  2. Once your identity is verified, you have access to your chart.
  3. Use the following link to access your chart:
